The Centers for Disease Control and Prevention (CDC) fall prevention initiative, Stopping Elderly Accidents, Deaths and Injuries, or STEADI, provides healthcare providers resources for falls risk screening and intervention. The general resources can be found here. The CDC STEADI website has additional discipline specific screening recommendations.
Additionally, the Dartmouth Health Geriatric Center of Excellence developed, with CDC’s permission, a community base falls risk screening tool, based on the CDC professional screening tool. This tool is called Balance Days. Balance Days training is incorporated with our Tai Ji Quan: Moving for Better Balance® training supported by NH DHHS CDC grant funding.
